Common Pricing Models
- Per patient, per month — a flat fee for each actively-enrolled patient, common for software-only platforms.
- Per provider — a flat monthly fee per billing provider or user seat, regardless of patient volume.
- Per location — a fee tied to practice location, sometimes combined with per-patient or per-provider pricing.
- Per program — separate pricing for each program (CCM, RPM, APCM) rather than one bundled rate.
- Percentage of collected revenue — common with full-service/managed offerings, where the vendor's own staff performs outreach and documentation.
Setup and Implementation Costs
Some vendors charge a one-time setup or implementation fee in addition to ongoing subscription pricing. Ask whether this is included, and what it covers (data migration, configuration, training).
Device Costs for RPM/RTM Programs
Remote monitoring programs typically involve a connected device cost — either purchased, provided by the vendor as part of the subscription, or billed separately. Clarify who owns/maintains the device and how device costs are structured before comparing RPM vendor pricing.
What Public Pricing Data Shows
Most CCM/RPM software vendors don't publish full pricing publicly, but some list a starting price. Based on publicly available listings reviewed on November 4, 2026:
These figures come from third-party software-directory listings, not from ChronicSync's own pricing research or a vendor's own site — verify current pricing directly with each vendor before relying on it. Directory-listed starting prices often exclude setup fees, minimum volume requirements, or program-specific add-ons.

- Is there a setup or implementation fee, and what does it include?
- Is there a minimum patient-volume commitment?
- What happens to pricing if we add a second program?
- For RPM/RTM: who owns and maintains the connected devices, and what do they cost?
- What's the contract length, and what does cancellation look like?
