Types of Solutions
Care management vendors generally fall into a few structural categories:
- Software-only platforms — you provide the platform's connective tissue (enrollment, care plans, task management), and your own clinical staff performs the care-management work.
- Fully-managed / outsourced services — the vendor's own staff performs patient outreach and documentation on your behalf, typically at a higher per-patient cost.
- Hybrid models — a mix, where the vendor provides some staffing support (e.g., initial outreach) while your team handles ongoing clinical work.
- EHR-native modules — care-management functionality built into or sold as an add-on to your existing EHR, trading flexibility for tighter integration.
Match Solution Type to Practice Size
A practice with a dedicated care manager and clinical capacity to run the program in-house often gets more value from a software-only platform. A practice without spare staff capacity may find a fully-managed service more realistic to launch quickly — see our Care Manager Capacity Calculator to estimate what your existing team can support before deciding.
Program Support
Confirm which specific programs (CCM, Complex CCM, RPM, RTM, APCM, PCM) each alternative actually supports, and whether that support is native or a bolt-on. Some vendors specialize in one program; others support several under one platform.
Pricing Models
See our pricing guide for the common models (per-patient, per-provider, percentage-of-revenue) — comparing alternatives on price alone without normalizing for pricing model can be misleading.
Implementation Differences
Ask each alternative what a realistic go-live timeline looks like for a practice your size, and what data migration support is included — implementation approach varies significantly and affects how quickly you can actually start billing.
Build vs. Buy
Building custom software is occasionally viable for large health systems with dedicated engineering resources, but it's rarely cost-effective for independent and mid-size practices given the ongoing maintenance required to track changing Medicare billing rules. For most practices, buying (or subscribing to) an existing platform is the more realistic path.
Evaluation Checklist
Use a consistent set of questions across every alternative you evaluate — see our free checklist for a ready-made list organized by clinical workflow, program support, documentation, billing, security, and support.
Where ChronicSync Fits
ChronicSync is a software-only platform supporting CCM, RPM, and APCM care-team workflows — patient management, care plans, remote monitoring, care coordination, patient engagement, and reporting in one connected system. It's built for practices that want their own clinical staff operating the program with a purpose-built platform, rather than outsourcing care-management staffing to a managed-service vendor.
