Remote Therapeutic Monitoring Reimbursement

RTM follows a payment structure similar to RPM's — a one-time setup code, a recurring device-supply code, and recurring treatment-management codes — under the Medicare Physician Fee Schedule.

Current Reimbursement Structure

RTM is billed once per episode of care for setup/education, then recurring monthly for device supply and, separately, for treatment-management time — mirroring RPM's three-component model but under its own code family.

Rate Variation

Like all Physician Fee Schedule amounts, RTM payment is adjusted by locality (Geographic Practice Cost Index) and updated at least annually — a national figure is a reference point, not a guarantee.

Device/Data Components

RTM's device-supply component covers data related to musculoskeletal or respiratory system status, which can include self-reported information in addition to device-transmitted data.

Treatment Management

RTM treatment-management billing requires monthly qualifying time and at least one interactive communication with the patient or caregiver, the same structure as RPM's treatment-management requirement.

Future Rule Changes

Proposed: CMS's CY2027 Proposed Rule would require RTM services only for established patients, require a separately billable initiating visit for RTM, and restrict RTM staffing to direct employees rather than contractors. CMS is also seeking comment (not proposing) on consolidating RPM/RTM codes into 4 new HCPCS codes. None of this is finalized.

Frequently Asked Questions

The structure is similar (setup, device supply, treatment management), but RTM uses its own separate codes and payment amounts — they aren't identical figures.

CMS has proposed operational changes (established-patient requirement, initiating visit, employment restriction) for CY2027, but hasn't finalized specific new RTM payment amounts as part of those proposals in the materials reviewed for this guide.
