Remote Patient Monitoring Codes Explained

RPM is billed through a small family of related codes covering setup, ongoing device supply, and treatment management — here's how they relate, summarized from CMS guidance rather than the proprietary CPT code book.

Current-Year Status

Current: These four codes and their general structure reflect CMS's current (CY2026) guidance per the CMS Telehealth & Remote Monitoring MLN Booklet (MLN901705, December 2025).

Proposed Future Changes

Proposed: CMS's CY2027 Proposed Rule (July 14, 2026) is seeking comment — but has not proposed — on consolidating the current 17 RPM/RTM CPT codes (including the four above) into 4 new HCPCS codes. This is not a finalized change.

How These Codes Relate to Each Other

99453 and 99454 cover the device side of the program (setup, then ongoing supply and data collection); 99457 and 99458 cover the clinical review and management side. A typical enrolled patient generates 99454 monthly for as long as they're actively monitored, plus 99457 (and sometimes 99458) in months where qualifying treatment-management time and an interactive communication actually occur.

Documentation Considerations

Document the total qualifying time for treatment-management codes, the interactive communication that occurred, and — for the device-supply code — evidence that data was collected and transmitted electronically to a secure location for the billing practitioner's review.

Frequently Asked Questions

In original, non-verbatim summary: it's the RPM code covering supply of a connected monitoring device along with data collection and transmission, billed on a recurring basis. See our Official Resources for CMS's own guidance on the exact code descriptor.

No. This page summarizes each code's general purpose and relationship in original language for education — the exact CPT code descriptors are proprietary to the American Medical Association.

CMS is only seeking comment on a possible future code-consolidation option as of the CY2027 proposed rule — nothing has been finalized or proposed as a firm change yet.
