This uses the insurer's allowed amount, not the billed charge, and assumes the deductible applies first with no copay in this simple mode. Noncovered services, balance billing, secondary insurance, and family accumulators are not modeled. This is not an insurer adjudication or financial advice.

How it's calculated
deductible applied = min(allowed, deductible remaining)patient = deductible applied + (allowed − deductible applied) × coinsurance, capped at the out-of-pocket remaining if entered
Worked example
Allowed $1,000, deductible remaining $200, 20% coinsurance, OOP remaining $10,000: patient $360, insurer $640.
