Chronic Care Management CPT Codes

Chronic Care Management is billed through a small family of related codes, each covering a different amount or type of qualifying time. Here's how they relate to each other, summarized from CMS guidance — not the proprietary CPT code book.

How These Codes Relate

CMS groups CCM billing into two tracks: non-complex CCM, billed by time thresholds for clinical staff or for the billing practitioner personally, and complex CCM, which additionally requires moderate- to high-complexity medical decision-making. A patient is billed under one track or the other for a given month, never both.

Non-Complex vs Complex CCM

Non-complex CCM (99490/99439, or 99491/99437 for personally-performed time) is the standard monthly track for most enrolled patients. Complex CCM (99487/99489) applies only when the patient's care that month genuinely requires 60+ minutes of clinical staff time and moderate- to high-complexity medical decision-making by the billing practitioner. See our CPT 99490 Reimbursement Guide for the base code in detail.

Clinical Staff vs Personally-Furnished Codes

99490/99439 and 99487/99489 describe clinical staff time performed under the billing practitioner's general supervision. 99491/99437 count only time the billing practitioner personally spends — clinical staff time never counts toward that pair's threshold.

Related Program Codes

Advanced Primary Care Management (G0556/G0557/G0558) bundles CCM-like activity into a flat monthly payment instead of separate time-based codes — see our APCM vs CCM comparison. Principal Care Management (99424-99427) and Transitional Care Management (99495/99496) are separate, related programs outside the scope of this page.

Frequently Asked Questions

99490 (clinical staff) or 99491 (personally performed by the billing practitioner) is the base code most enrolled patients are billed under each month, depending on who performs the qualifying work.

The codes describe different performers of the qualifying time (clinical staff vs. the billing practitioner personally) for the same underlying non-complex CCM service — confirm current same-month combination rules with your coding advisor or Medicare Administrative Contractor before billing both.
