Updated for 2026. CPT 99489 is an add-on code for Complex Chronic Care Management (CCM). It is used when qualifying complex CCM services extend beyond the initial time represented by CPT 99487.
The key point is simple: CPT 99489 cannot stand alone. The patient must first meet the requirements for CPT 99487, including the initial complex CCM time requirement and moderate- or high-complexity medical decision-making by the billing practitioner.
What Is CPT 99489?
CMS identifies CPT 99489 as the add-on code for each additional 30 minutes of qualifying clinical-staff Complex Chronic Care Management services during a calendar month.
It is reported with CPT 99487 when the patient's documented complex CCM activity exceeds the base service and satisfies the applicable additional-time requirements.
CPT 99487 vs CPT 99489
CodeServiceTime99487Base Complex CCMFirst 60 minutes of qualifying clinical-staff time99489Additional Complex CCMEach additional 30 minutes
CPT 99489 is therefore not an alternative to CPT 99487. It extends the complex CCM service when additional qualifying time is supported.
What Makes Complex CCM Different?
Complex CCM is more than ordinary CCM with additional minutes.
CMS states that complex CCM requires:
- Two or more qualifying chronic conditions
- A comprehensive patient-centered care plan
- Qualifying clinical-staff care-management time
- Ongoing oversight and management by the billing practitioner
- Moderate- or high-complexity medical decision-making by the physician or other qualified healthcare professional
That medical decision-making requirement applies to the complex CCM service and is an important distinction from standard CCM.
When Does CPT 99489 Apply?
CPT 99489 may be considered after the requirements for CPT 99487 have been satisfied and the practice documents sufficient additional qualifying complex CCM time.
Example
A patient with heart failure, chronic kidney disease, COPD, and diabetes experiences medication changes and worsening symptoms during the month.
The care team performs:
- Medication reconciliation
- Specialist coordination
- Post-discharge follow-up
- Patient and caregiver communication
- Care-plan revision
- Clinical escalation to the billing practitioner
The billing practitioner also documents the required moderate-complexity medical decision-making.
If the qualifying complex CCM time supports the base CPT 99487 service and an additional qualifying 30-minute period, the practice may evaluate CPT 99489 as an add-on code.
CPT 99489 Cannot Be Billed by Itself
CPT 99489 is an add-on code. The record must first support the applicable base Complex CCM service.
A claim should not report 99489 simply because 30 minutes of care-management activity occurred. The documentation must support CPT 99487 and the additional qualifying time represented by 99489.
What Activities May Contribute to Complex CCM Time?
Qualifying activity depends on the patient's care plan and documented clinical needs. Examples may include:
- Medication management and reconciliation
- Reviewing changes in chronic-condition status
- Care-plan establishment or substantial revision
- Coordination with specialists
- Hospital or post-acute transition follow-up
- Communication with patients and caregivers
- Coordination with home-health or community resources
- Managing significant barriers affecting treatment
- Escalating clinically important findings
- Implementing practitioner-directed care-plan changes
Purely clerical work, waiting time, or unrelated administrative activity should not automatically be included as qualifying CCM time.
Complex CCM Requires Meaningful Practitioner Involvement
Although clinical staff may perform much of the qualifying care-management work, Complex CCM cannot operate independently of the billing practitioner.
The practitioner remains responsible for the required oversight, management, and moderate- or high-complexity medical decision-making.
The medical record should make that involvement clear.
Care Plan Requirements
Complex CCM includes comprehensive care planning. CMS materials describe the care plan as patient-centered and capable of addressing elements such as:
- Chronic-condition problem list
- Expected outcomes and prognosis
- Measurable treatment goals
- Medication management
- Symptoms
- Planned interventions
- Functional and cognitive needs
- Psychosocial or environmental needs
- Caregiver involvement
- Coordination with outside practitioners and resources
For complex CCM, the month's documented work should support the intensity and complexity of the care being provided.

If the documentation also supports all other requirements for complex CCM, including the required practitioner medical decision-making, the billing team can evaluate whether the base 99487 service and an additional 99489 unit are supported under current coding rules.
Do Not Round Up Unsupported Time
Practices should use the actual qualifying time documented during the calendar month and apply the current CPT time rules.
Do not automatically round incomplete additional time up to a full CPT 99489 unit. The claim should reflect the service actually supported by the documentation.
Can More Than One Unit of 99489 Be Reported?
CPT 99489 describes additional complex CCM time. Whether multiple units are reportable depends on the documented qualifying time, current CPT instructions, Medicare medically unlikely edits, and payer-specific rules.
Practices should verify current CMS and Medicare Administrative Contractor guidance before reporting multiple units.
99489 vs 99439
Both are add-on CCM codes, but they belong to different care-management pathways.
CodeCare TypeAdditional Time99439Standard CCMEach additional 20 minutes99489Complex CCMEach additional 30 minutes
CPT 99439 is associated with CPT 99490. CPT 99489 is associated with CPT 99487.
Can 99489 Be Reported With 99490?
CMS does not allow standard and complex CCM to be reported together for the same patient during the same calendar month.
Practices should determine whether the documented month's services support the standard CCM pathway or the complex CCM pathway rather than mixing their base and add-on code families.
Can 99489 Be Reported With 99491?
CMS states that practitioner-only CCM codes such as CPT 99491 and 99437 are not reported during the same calendar month as clinical-staff CCM codes including 99487 and 99489.
Accurate tracking of practitioner and clinical-staff work is therefore important before billing.
Complex CCM Documentation Example
Example: Patient with CHF, COPD, CKD, and diabetes experienced worsening dyspnea and medication changes following hospital discharge. Clinical staff reconciled discharge medications, coordinated cardiology and nephrology recommendations, contacted patient and caregiver regarding symptoms and adherence, substantially revised the comprehensive care plan, and coordinated follow-up. Billing practitioner reviewed the changing clinical picture and documented moderate-complexity medical decision-making. Total qualifying complex CCM time documented for the month: 92 minutes.
This type of documentation gives a reviewer more context than simply recording 99487 + 99489 completed.
Common CPT 99489 Billing Mistakes
1. Billing 99489 Without 99487
99489 is an add-on code and requires the applicable base complex CCM service.
2. Looking Only at the Time Total
Complex CCM also requires moderate- or high-complexity medical decision-making and other CCM service elements.
3. Counting Administrative Time
Purely clerical work should not automatically increase the complex CCM time total.
4. Mixing Standard and Complex CCM Codes
Do not combine the standard CCM and complex CCM code families for the same patient and month when CMS rules prohibit concurrent reporting.
5. Missing Practitioner Medical Decision-Making
A large amount of clinical-staff time does not by itself establish Complex CCM. The practitioner medical decision-making requirement must also be satisfied.
6. Double Counting Time
When other care-management or remote-monitoring services are furnished, practices should make sure the same work and time are not improperly used to support multiple claims.
